Sex करने पर लिंग में दर्द होना कोई normal बात नहीं है। कभी-कभी हल्का discomfort dryness, friction या पहली बार संबंध बनाने की वजह से हो सकता है, लेकिन अगर pain बार-बार हो रहा है, लिंग की चमड़ी पीछे नहीं जा रही, लिंग की सील कट रही है, जलन या सूजन है, erection में लिंग टेढ़ा हो रहा है, या sex के दौरान अचानक “crack/pop” जैसी आवाज के साथ तेज दर्द हुआ है, तो इसे ignore नहीं करना चाहिए।

पुरुषों में sex के दौरान लिंग में दर्द के 5 common और important कारण हैं: Phimosis, Tight Frenulum, Balanitis, Peyronie’s Disease और Penile Fracture। इनमें से कुछ conditions धीरे-धीरे बढ़ती हैं और doctor consultation से ठीक हो सकती हैं, जबकि penile fracture emergency है और इसमें तुरंत urologist या emergency care की जरूरत होती है। Cleveland Clinic के अनुसार penile fracture में अक्सर pop/crack sound, अचानक erection खत्म होना, तेज pain, swelling और bruising हो सकती है, और समय पर इलाज जरूरी होता है।

अगर sex के दौरान pain के साथ bleeding, repeated cuts, foreskin tightness, redness, discharge, bad smell, painful erection, penile bend, lump या urinary symptoms हैं, तो proper diagnosis जरूरी है। इस ब्लॉग में हम sex के दौरान लिंग में दर्द के 5 मुख्य कारण, उनके symptoms, diagnosis, treatment options और doctor को कब दिखाना चाहिए – सब कुछ detail में समझेंगे।

Sex करने पर लिंग में दर्द क्यों होता है?

Sex के दौरान penis पर friction, stretching, pressure और erection-related tension होता है। अगर foreskin tight है, frenulum छोटा है, infection है, penis में scar tissue है या sex के दौरान injury हुई है, तो pain हो सकता है।

लिंग में दर्द इन जगहों पर महसूस हो सकता है:

लिंग के head यानी glans पर
foreskin यानी चमड़ी में
frenulum यानी लिंग की सील में
shaft यानी लिंग के middle part में
erection के दौरान अंदर खिंचाव जैसा
intercourse के बाद जलन या soreness के रूप में

Pain का pattern cause समझने में मदद करता है। जैसे skin पीछे न जाने पर phimosis, seal कटने पर frenulum problem, redness/itching में balanitis, painful bend में Peyronie’s disease और अचानक चोट में penile fracture का शक हो सकता है।

1. Phimosis: लिंग की चमड़ी टाइट होना

Phimosis में लिंग की foreskin यानी चमड़ी glans के पीछे पूरी तरह नहीं जाती। कई पुरुषों में यह समस्या sex के दौरान ज्यादा महसूस होती है क्योंकि erection में penis size बढ़ता है और tight foreskin stretch होती है।

BAUS के अनुसार tight foreskin में steroid cream कभी-कभी mild scarring में help कर सकती है, infection हो तो antibiotics की जरूरत हो सकती है, और scarring/BXO जैसे cases में ZSR Circumcision treatment हो सकता है।

Phimosis में Sex के दौरान Pain क्यों होता है?

जब foreskin tight होती है, तो intercourse के दौरान skin पीछे खिंचती है लेकिन खुलती नहीं। इससे:

skin में खिंचाव
दर्द
छोटे cuts
redness
swelling
condom लगाने में difficulty
intercourse में burning या tightness
sex के बाद foreskin में सूजन हो सकती है।

Phimosis के लक्षण

चमड़ी पूरी पीछे नहीं जाती
erection में चमड़ी ज्यादा tight लगती है
glans expose नहीं होता
sex में दर्द या खिंचाव
बार-बार redness या infection
urine करते समय ballooning
cleaning में problem
sex के बाद swelling

Phimosis का इलाज

Treatment severity पर depend करता है:

Mild cases में gentle stretching और steroid cream
Infection हो तो पहले infection treatment
Recurrent infection या scarring में circumcision
Selected cases में partial circumcision या foreskin-preserving options
अगर phimosis के साथ frenulum tight है, तो frenulum भी साथ में manage हो सकता है

Phimosis को forcefully pull नहीं करना चाहिए। इससे cuts, bleeding और paraphimosis का risk हो सकता है।

2. Tight Frenulum: लिंग की सील टाइट होना

लिंग के नीचे की तरफ foreskin और glans को जोड़ने वाली छोटी tissue band को frenulum कहते हैं। आम भाषा में कई लोग इसे लिंग की सील या link ki seal कहते हैं।

Tight frenulum में foreskin पीछे जा सकती है, लेकिन नीचे की seal tight होकर skin को रोकती है, खिंचती है, pain देती है या cut हो जाती है। Transcript में Dr. Vijayant Govinda Gupta बताते हैं कि अगर skin पीछे जाने के बाद seal tight होकर penis head को नीचे खींचती है, insertion में pain करती है, या जोर लगने पर कटकर bleeding करती है, तो यह tight frenulum की problem हो सकती है।

Tight Frenulum में Sex के दौरान Pain क्यों होता है?

Sex के दौरान जब foreskin पीछे जाती है, तो tight frenulum glans को नीचे खींच सकता है। इससे penis bend हो सकता है और insertion painful हो सकता है। अगर force ज्यादा लगे तो frenulum tear हो सकता है।

Sex में लिंग की सील पर pain
Seal का बार-बार कटना
Bleeding during sex
Foreskin पीछे जाती है लेकिन नीचे खिंचाव होता है
Penis head नीचे की तरफ bend होता है
Insertion में difficulty
Healing के बाद फिर उसी जगह tightness
Pain या fear की वजह से early discharge

Transcript में यह भी बताया गया है कि अगर frenulum normal है और कोई pain, bleeding, cut या sex difficulty नहीं है, तो treatment की जरूरत नहीं होती; लेकिन repeated cut, bleeding, pain, early discharge, penis bend या sex difficulty हो तो treatment consider किया जा सकता है।

Tight Frenulum और Phimosis में फर्क

यह फर्क समझना बहुत जरूरी है:

Tight frenulum: Skin पीछे चली जाती है लेकिन last में नीचे की seal pain या bending करती है।
Phimosis: Skin पूरी पीछे ही नहीं जाती या glans के पीछे tight ring बनती है।

कई patients में दोनों साथ भी हो सकते हैं। ऐसे cases में doctor decide करते हैं कि केवल frenuloplasty चाहिए या circumcision/partial circumcision भी जरूरी है।

Tight Frenulum का इलाज

Tight frenulum का treatment symptoms पर depend करता है:

अगर कोई symptom नहीं है, treatment की जरूरत नहीं
बार-बार कटना या bleeding हो तो frenuloplasty
Phimosis साथ हो तो circumcision/partial circumcision के साथ frenulum management
Wrong cutting या self-treatment से बचना जरूरी है

Frenuloplasty एक छोटा procedure हो सकता है, लेकिन यह area sensitive होता है, इसलिए trained surgeon से ही करवाना चाहिए।

3. Balanitis: लिंग के Head या चमड़ी में Infection/सूजन

Balanitis में glans यानी लिंग के head में सूजन, redness, irritation या infection हो सकता है। अगर foreskin भी involved हो, तो इसे balanoposthitis कहा जा सकता है। यह condition uncircumcised men, diabetes patients, poor hygiene, fungal infection या repeated irritation में ज्यादा दिख सकती है।

NICE के अनुसार balanitis में penile discomfort, itching, bleeding, skin splitting, discharge, urinary symptoms या sexual dysfunction हो सकती है; साथ ही glans/foreskin में redness, swelling, discharge, fissuring या ulceration हो सकता है।

Balanitis में Sex के दौरान Pain क्यों होता है?

जब glans या foreskin inflamed होती है, तो sex के दौरान friction से pain, burning और redness बढ़ सकती है। अगर skin में cracks हैं, तो intercourse बहुत painful हो सकता है।

Balanitis के लक्षण

लिंग के head पर redness
itching या burning
foreskin के नीचे bad smell
सफेद discharge या जमा हुआ material
sex के दौरान burning pain
urine करते समय जलन
skin cracks या छोटे cuts
glans पर swelling
repeated fungal infection

Balanitis के Common कारण

fungal infection
bacterial infection
diabetes
poor hygiene
tight foreskin
harsh soap या chemical irritation
condom/lubricant allergy
sexually transmitted infection
excessive moisture

Balanitis का इलाज

Balanitis का इलाज cause पर depend करता है:

fungal infection में antifungal cream
bacterial infection में antibiotics
allergy/irritation में triggering product बंद करना
diabetes control
gentle hygiene
recurrent cases में phimosis evaluation
STI doubt हो तो testing और partner evaluation

Self-medication से बचें, क्योंकि गलत cream infection को बढ़ा सकती है या skin को damage कर सकती है।

4. Peyronie’s Disease: Erection में दर्द और लिंग टेढ़ा होना

Peyronie’s disease में penis के अंदर scar tissue या plaque बनता है। इससे erection में pain, curvature, shortening, narrowing या sex difficulty हो सकती है। Mayo Clinic के अनुसार Peyronie’s disease में fibrous scar tissue penis की deeper tissues में बनता है, जिससे curved और painful erections हो सकती हैं।

Peyronie’s Disease में Sex के दौरान Pain क्यों होता है?

जब erection आता है, तो scarred area normal tissue की तरह stretch नहीं करता। इससे penis bend हो सकता है और painful erection हो सकती है। अगर curvature ज्यादा है, तो intercourse difficult या painful हो सकता है।

Peyronie’s Disease के लक्षण

Erection में pain
लिंग का ऊपर, नीचे या side में bend होना
penis shaft में hard lump/plaque feel होना
erection में narrowing या hourglass shape
penis छोटा लगना
sex में penetration difficulty
erectile dysfunction
psychological stress

Mayo Clinic के अनुसार Peyronie’s disease में acute phase के दौरान curvature या length changes worsen हो सकते हैं और erection pain हो सकता है; chronic phase में symptoms stable हो जाते हैं।

Peyronie’s Disease का कारण

Peyronie’s disease का exact cause हमेशा clear नहीं होता, लेकिन repeated micro-trauma, intercourse injury, genetics, diabetes, connective tissue tendency और age-related factors role play कर सकते हैं।

Peyronie’s Disease इलाज

Treatment Stage पर Depend करता है:

Early/acute stage में observation, medicines, pain control
Curvature significant हो तो injections या traction therapy selected cases में
Stable Severe Curvature में Surgery
Peyronie’s + ED में Penile Implant Selected Cases में

अगर Erection में नया Bend, Pain या Lump महसूस हो, तो जल्दी Urologist/Andrologist को दिखाना बेहतर है।

5. Penile Fracture: Sex के दौरान अचानक चोट वाली Emergency

Penile fracture sex के दौरान लिंग में अचानक injury की emergency condition है। यह तब हो सकता है जब erect penis जोर से bend हो जाए या गलत angle से pressure लगे। इसमें penis के अंदर tunica albuginea नाम की covering tear हो सकती है।

Cleveland Clinic के अनुसार penile fracture में अक्सर cracking/popping sound, erection का तुरंत खत्म होना, तेज pain, swelling और bruising होती है। Diagnosis physical examination से हो सकती है, लेकिन ultrasound/MRI या urethral evaluation भी जरूरत के अनुसार हो सकता है।

Penile Fracture के लक्षण

Sex के दौरान sudden तेज pain
“Pop” या “crack” जैसी आवाज
erection अचानक खत्म होना
penis में swelling
blue/black bruising
penis deformity
urine में blood
urine करने में difficulty
severe tenderness

यह Emergency क्यों है?

Penile fracture को delay करने से complications हो सकती हैं:

permanent curvature
erectile dysfunction
painful erection
scar formation
urinary injury
sexual difficulty

Mayo Clinic के अनुसार untreated penile fracture curved penis या erectile dysfunction जैसी problems का कारण बन सकता है।

Penile Fracture का इलाज

Penile fracture में आमतौर पर urgent surgical repair की जरूरत होती है। Painkiller लेकर घर पर wait करना risk बढ़ा सकता है। अगर sex के दौरान अचानक crack sound और swelling हुई है, तो तुरंत emergency/urologist के पास जाएं।
